Temporary Full Time Patient Services Coordinator (maternity leave)
Job Objectives:
- To provide direct support to people living with, and affected by ALS.
- To implement and execute the patient service programs of the Society.
- To source additional local support services in communities throughout BC and the Yukon that could provide additional assistance to people affected with ALS.
- Support the Patient Services Manager
Responsibilities:
- Contacts ALS patients by phone to offer personalized support.
- Builds relationships with families living with ALS
- Book transportation/accommodation for people living with ALS attending medical appointments
- Assists the Patient Services Manager in the coordination and implementation of;
– Day of Caring for Caregivers (5 locations) – October dates
– Camp Alohi Lani
– Psychological Treatment Program
– Organizing Mobile Clinics (province wide) - Ensures that patient services of the Society are communicated to people living with ALS, their families and caregivers and to health care professionals
- Resource advocate for people living with ALS and their families
- Management of ALS patient database and records
– Prepares ALS membership application form and package
– Encodes patient data in Raiser’s Edge database - Performs duties are per patient services policies and procedures.
Qualifications:
- Experience in healthcare setting in the field of counseling or social work an asset
- Ability to handle multiple projects at the same time
- Must possess excellent verbal communication skills
- Must be an innovative thinker with strong abilities to problem solve
- Must be proficient in Windows-based programs
- Must have valid BC driver’s license (Class 5) and access to a vehicle (mileage will be compensated)
- Positive and mature disposition
- Excellent team player, creative, flexible and detail oriented
